NYS PRI
The Patient Review Instrument (PRI) is an assessment tool required by New York State Department of Health when applying for any Residential Health Care Facility (RHCF) for skilled long term placement. It provides information to facilities about your family member’s current medical condition and ensures that the facility can provide the proper care needed. The PRI is valid for 30 days as long as there is no change in condition. Once completed, it will be sent to the facilities of your choice. This service can only be provided by a PRI Certified Registered Nurse.
Screen Assessments
The Screen Assessment is an added component to the PRI. It is also required for prospective residents that live in the community, applying for Residential Health Care Facility (RHCF) for skilled long term placement. The screen has two purposes. It will determine if the person has the ability to be cared for in any other setting besides a long term facility. It also assesses for mental illness, intellectual disability, or developmental disability.
